Birth records are not public records and may only be issued to those persons entitled to receive certified copies.
Plan ahead and allow extra time when requesting a birth certificate of your child born at Sarah Bush Lincoln Health Center, or in Mattoon.
Complete the Birth Certificate Application below and obtain a legible copy (clearly readable) of your valid photo identification card. Send both of these items along with the payment required as described in the application, to the address below.

IMPORTANT: Due to the COVID-19 Pandemic, City Hall is closed to the public. You can drop off an envelope with the required items described above in the Finance Department water billing mailbox just to the east of the City Hall parking lot.
In your application, be certain to include a telephone number where you can be reached between the hours of 8:00 a.m. and 4:00 p.m.
Since all birth certificate applications are processed and returned by mail, allow several days for requests to be completed and returned to you. Current processing times are within two business days, upon validation of application. For expedited delivery, appropriate delivery fees will be charged.
In all instances, if identification is not provided, unreadable, or expired, your request will be returned unprocessed.
If you do not have a valid photo identification card, you must provide TWO pieces of documentation with your name, one of which must show your current address, to prove your identity.
A. One documentation may be a bill or other mail.
B. The other documentation must be one of the following items:
- Medical/car insurance card
- Car registration paperwork
- Credit card statement
- Bank statement
- Paycheck stub with imprinted information
- Public assistance card
- Voter’s registration card
- Active duty military ID with issued & expiration dates
